This is the registry Scribeable reads while it writes a note. 236 of the 239 entries carry a scorer: for 234 of them the score is computed in code from the documented inputs rather than recalled by the model, and 2 (PERC Rule and PREVENT 10-Year Total CVD Risk (AHA)) are scored only on the interactive form; the other 3 are carried for activation and documentation guidance and scored in their external tools. Each page lists the inputs the note needs, the scoring criteria, the interpretation bands, the sources in the citation registry, and exactly how the note pipeline handles that score.
