Coding with the sentence that supports it
ICD-10 codes, an E/M level and HCC V28 codes are suggested from the note, each tied to the line in the note that supports it. The HCC crosswalk has 8,400 entries. You decide what goes on the claim.

When the visit gives it the inputs, Scribeable computes scores like PHQ-9, GAD-7, AUDIT-C, FIB-4 and STOP-BANG in code, not recalled by the model. A stated score that disagrees is flagged for you to correct.

The after-visit summary is drafted from the note you finished, as a PDF or an email. Referral letters, work and school notes, and disability and FMLA support letters come from the same note. Included on Pro.
