When it activates
Scribeable activates a calculator from the encounter. The Berlin Criteria for ARDS can activate when the transcript or pasted chart context documents any of these:
- ARDS diagnosis
- Acute respiratory failure classification
- Bilateral infiltrates with hypoxemia
- Ventilator management in ARDS
- ARDS severity assessment
A coded diagnosis can also activate it. ICD-10 prefixes carried by the registry: J80.
Note-type scope: it activates on notes whose subject resolves to critical care, pulmonology, emergency medicine. A note that resolves to another subject does not carry it even when the clinician's specialty matches; a note with no resolvable subject (a generic consultation) falls back to the specialty match.

Requires: onset within 1 week, bilateral opacities, not fully explained by cardiac failure, PEEP ≥5. Severity by P/F ratio: Mild 200-300, Moderate 100-200, Severe <100.

Interpretation
Mild (P/F 200-300): 27% mortality. Moderate (P/F 100-200): 32% mortality, consider prone positioning. Severe (P/F <100): 45% mortality, prone positioning recommended, consider ECMO.
