When it activates
Scribeable activates a calculator from the encounter. The Sodium Corrected for Hyperglycemia can activate when the transcript or pasted chart context documents any of these:

- DKA sodium assessment
- HHS sodium correction
- Severe hyperglycemia with electrolyte abnormalities

Scoring
Corrected Na = Measured Na + 1.6 × ((glucose − 100) / 100) for glucose 100–400 mg/dL; use correction factor 2.4 for glucose >400 mg/dL
Corrected Na = Measured serum sodium in mEq/L + 1.6 × ((Serum glucose in mg/dL − 100) / 100)
Interpretation
If corrected Na is normal (135–145): hyponatremia is dilutional from hyperglycemia alone. If corrected Na remains low: true hyponatremia coexists. If corrected Na is elevated: hypernatremia masked by glucose-induced water shift — monitor closely during glucose correction.
