Scoring
Equivalent doses: hydrocortisone 20mg = prednisone 5mg = prednisolone 5mg = methylprednisolone 4mg = triamcinolone 4mg = dexamethasone 0.75mg = betamethasone 0.6mg
Interpretation
Dexamethasone has longest half-life (36–72h) and highest potency. Hydrocortisone has most mineralocorticoid activity. Prednisone >20 mg/day for >3 weeks: taper required (risk of adrenal suppression). Stress dosing: hydrocortisone 100 mg IV, then 50 mg q8h.
