When it activates

Scribeable activates a calculator from the encounter. The Harvey-Bradshaw Index can activate when the transcript or pasted chart context documents any of these:

  • Crohn disease activity assessment
  • Crohn disease flare severity
  • Crohn treatment response monitoring
  • Crohn remission assessment

A coded diagnosis can also activate it. ICD-10 prefixes carried by the registry: K50.

  • Crohn's K50.00

Note-type scope: it activates on notes whose subject resolves to gi, hospital medicine. A note that resolves to another subject does not carry it even when the clinician's specialty matches; a note with no resolvable subject (a generic consultation) falls back to the specialty match.

Inputs the note needs

5 inputs. Each is resolved from the transcript, the pasted chart context and charted labs; at this calculator’s tier a missing input is never requested, and a missing required input keeps the score out of the draft.

InputTypeUnit / optionsGuidance
General well-being (0=very well, 1=slightly below par, 2=poor, 3=very poor, 4=terrible)symptom
Abdominal pain (0=none, 1=mild, 2=moderate, 3=severe)symptom
Number of liquid or very soft stools per daysymptom
Abdominal mass (0=none, 1=dubious, 2=definite, 3=definite and tender)exam
Number of complications: arthralgia, uveitis, erythema nodosum, aphthous ulcers, pyoderma gangrenosum, anal fissure, new fistula, abscess (1 point each)history

Reference for licensed clinicians. Where the registry text carries dose thresholds or conversion factors, they are as published in the cited sources. This page is not a prescribing tool and not individualised medical advice; verify against a current drug reference and the current guideline before acting. Medical disclaimer.

Scoring

General well-being (0-4) + Abdominal pain (0-3) + Number of liquid stools/day + Abdominal mass (0-3) + Complications (1 each). No upper bound.

Interpretation

<5: Remission. 5-7: Mild activity. 8-16: Moderate activity. >16: Severe activity.

How Scribeable handles it in the note

When an encounter activates the Harvey-Bradshaw Index (its highest tier is recommended), Scribeable does not ask you for missing inputs. The score is computed in code from whatever inputs resolve from the transcript, the pasted chart context or charted labs; a computed value is written into the draft only when every required input already resolves.

The score is computed in code from the resolved inputs, not recalled by the language model. A stated Harvey-Bradshaw Index value is compared against the computed one when the draft's structured summary carries it; a disagreement found there is flagged to the second drafting pass, which is instructed to correct the score and its interpretation, unless Scribeable has supplied the computed value to that pass, in which case the value the draft writes is checked against the supplied one and a detected difference is shown beside the note for you to adjudicate rather than silently rewritten.

Classified as a risk score: activated strictly, by a documented trigger, never by a permissive reading of the encounter.
