When it activates
Scribeable activates a calculator from the encounter. The 4Ts Score for Heparin-Induced Thrombocytopenia can activate when the transcript or pasted chart context documents any of these:

- Platelet drop during heparin therapy
- Suspected HIT evaluation
- New thrombosis on heparin
- Heparin exposure with falling platelets

Note-type scope: it activates on notes whose subject resolves to hospital medicine, critical care. A note that resolves to another subject does not carry it even when the clinician's specialty matches; a note with no resolvable subject (a generic consultation) falls back to the specialty match.

Scoring
Four categories, each scored 0-2: Thrombocytopenia (degree of platelet fall), Timing (of platelet fall onset), Thrombosis (or other sequelae), oTher causes (of thrombocytopenia). Total range: 0-8.

Interpretation
Score 0-3: Low probability (~5% HIT). May continue heparin, HIT antibody testing optional. Score 4-5: Intermediate probability (~14-25% HIT). Send HIT antibody (ELISA or SRA), consider switching to non-heparin anticoagulant. Score 6-8: High probability (~50-80% HIT). Discontinue all heparin, start non-heparin anticoagulant (argatroban, bivalirudin, fondaparinux), send confirmatory testing. Do NOT use warfarin until platelets recover.
