When it activates

Scoring
7 symptom questions, each scored 0 (not at all) to 5 (almost always). Maximum symptom score 35. Plus 1 quality of life question scored 0-6 (reported separately).
Interpretation
0-7: Mild symptoms — watchful waiting, lifestyle modifications. 8-19: Moderate symptoms — alpha-blocker (tamsulosin) or 5-alpha-reductase inhibitor (finasteride/dutasteride) or combination. 20-35: Severe symptoms — combination therapy, consider surgical intervention (TURP, HoLEP, UroLift, Rezum). QoL score guides patient satisfaction and treatment urgency. MCID: 3-point change.

The score is computed in code from the resolved inputs, not recalled by the language model. If the draft states a International Prostate Symptom Score (IPSS) value that disagrees with the computed one, the disagreement is flagged to the second drafting pass, which is instructed to correct the score and its interpretation before you see the note.
