Scoring

Two-axis system: Stage (depth) 0-III × Grade (complications) A-D. Stage 0: Pre/post-ulcerative epithelialized lesion. Stage I: Superficial wound not involving tendon, capsule, or bone. Stage II: Wound penetrating to tendon or capsule. Stage III: Wound penetrating to bone or joint. Grade A: Clean wound. Grade B: Non-ischemic infected wound. Grade C: Ischemic non-infected wound. Grade D: Ischemic infected wound. Score = stage number (0-3).

Interpretation

Stage 0A: Preventive care — offloading, education, regular foot exams. Stage IA: Superficial clean wound — offloading, moist wound care, high healing probability. Stage IB/IC: Requires antibiotics (B) or vascular assessment/revascularization (C). Stage ID: Worst prognosis — dual threat. Stage II-III: Deep wounds require surgical evaluation. Any Grade D: Highest amputation risk — urgent combined vascular and infectious disease management. Grade D at any stage has lowest healing rate and highest amputation rate (~50% major amputation).
