Scoring

Category 0: Incomplete — needs additional imaging. Category 1: Negative — routine screening. Category 2: Benign — routine screening. Category 3: Probably benign (<2% malignancy) — short-interval follow-up. Category 4: Suspicious (4A 2-10%, 4B 10-50%, 4C 50-95%) — tissue diagnosis recommended. Category 5: Highly suggestive of malignancy (>95%) — tissue diagnosis required. Category 6: Known biopsy-proven malignancy.

Interpretation

BI-RADS 0: Recall for diagnostic workup (spot compression, ultrasound, MRI). BI-RADS 1-2: Return to routine screening interval. BI-RADS 3: 6-month follow-up imaging, requires 2 years of stability to downgrade. BI-RADS 4A: Low suspicion biopsy — core needle preferred. BI-RADS 4B-C: Intermediate to high suspicion — biopsy indicated, concordance assessment essential. BI-RADS 5: Biopsy required — surgical consultation indicated. BI-RADS 6: Staging and treatment planning.
