EuroSCORE II
Predicts in-hospital mortality after cardiac surgery. Requires dedicated online calculator with 18 variables.
- Highest activation tier
- Recommended
- How it is scored
- Registry entry; scored in an external tool
- Scoring type
- external tool required
- Care settings
- Emergency department, Inpatient
When it activates
Scribeable activates a calculator from the encounter. The EuroSCORE II can activate when the transcript or pasted chart context documents any of these:
- Cardiac surgery mortality prediction
- European cardiac surgery risk
- Valve surgery risk assessment
- Combined cardiac procedure risk
Note-type scope: it activates on notes whose subject resolves to perioperative, cardiology, hospital medicine. A note that resolves to another subject does not carry it even when the clinician's specialty matches; a note with no resolvable subject (a generic consultation) falls back to the specialty match.
Scoring
Logistic model with 18 variables. Use official EuroSCORE II calculator at euroscore.org.
Interpretation
Low risk: <2%. Moderate: 2-5%. High: >5%. Used in European guidelines for cardiac surgery decision-making and TAVR eligibility.

How Scribeable handles it in the note
Scribeable carries the EuroSCORE II in its registry for activation and documentation guidance but does not compute it: the inputs live in the external tool, not in the note.
When an encounter activates it, the value written in the note is whatever you state or the model recalls: Scribeable neither computes it nor compares a stated value against a computed one.
Classified as a risk score: activated strictly, by a documented trigger, never by a permissive reading of the encounter.
