When it activates
Scribeable activates a calculator from the encounter. The Padua VTE Risk Assessment Score can activate when the transcript or pasted chart context documents any of these:
- Hospital admission VTE prophylaxis decision
- Immobilized medical patient
- Cancer patient hospitalization
- Thromboprophylaxis assessment
- Post-surgical VTE risk stratification
A coded diagnosis can also activate it. ICD-10 prefixes carried by the registry: I26, I82, Z87.111.

Interpretation
Score ≥4: High VTE risk — pharmacologic prophylaxis recommended (enoxaparin 40 mg SQ daily or heparin 5000 units SQ q8–12h) unless contraindicated. Score <4: Low VTE risk — mechanical prophylaxis (SCDs) generally sufficient. Always assess bleeding risk (IMPROVE or HAS-BLED) before initiating pharmacologic prophylaxis.
