Make Undifferentiated Reasoning Easy to Follow
Urgent care starts with limited information. A patient may arrive with a broad symptom, an incomplete medication list, scattered outside records, and no useful prior chart. The note has to show how the clinician framed the complaint, examined relevant systems, considered a sufficiently broad differential, and used available testing to reach the working assessment. Scribeable drafts that reasoning from what the clinician says and does during the encounter. Clinical judgment stays with the clinician. After the draft pass, a separate verification pass reviews ambiguity in the audio or chart context and asks a clarification question when needed. That step matters when interruptions, overlapping speech, absent history, or an uncertain medication name leaves a detail unresolved.
