Scoring

Stage I: Confined to organ of origin. Stage II: Extension beyond organ but within pelvis. Stage III: Extension to pelvic sidewall, lower vagina, or regional lymph nodes. Stage IV: Extension to distant organs (IVA: bladder/rectum invasion; IVB: distant metastases). Substages vary by cancer type. Score = major stage number (1-4).

Interpretation

Stage I: Generally favorable prognosis — surgery often curative (cervical IA1: conization; endometrial IA G1-2: hysterectomy). Stage II: Combined modality therapy often needed — surgery + radiation (cervical IIB: chemoradiation). Stage III: Multimodality therapy — neoadjuvant chemotherapy + surgery + radiation for many types. Stage IV: Systemic therapy primary — palliative intent for IVB, multimodal for IVA. 5-year survival varies dramatically: cervical IA >95%, cervical IVB <15%; ovarian IA >90%, ovarian IV <20%. 2018 FIGO update incorporated imaging and pathology findings for cervical cancer staging.
