Also written as: mMRC, mMRC dyspnea, MRC dyspnea scale.

When it activates
Scribeable activates a calculator from the encounter. The Modified Medical Research Council Dyspnea Scale can activate when the transcript or pasted chart context documents any of these:
- COPD symptom assessment
- Dyspnea severity grading
- COPD treatment escalation
- Pulmonary rehabilitation referral
A coded diagnosis can also activate it. ICD-10 prefixes carried by the registry: J44.
Note-type scope: it activates on notes whose subject resolves to pulmonology, hospital medicine. A note that resolves to another subject does not carry it even when the clinician's specialty matches; a note with no resolvable subject (a generic consultation) falls back to the specialty match.
Inputs the note needs
1 input. Each is resolved from the transcript, the pasted chart context and charted labs; at this calculator’s tier a missing input is never requested, and a missing required input keeps the score out of the draft.
| Input | Type | Unit / options | Guidance |
|---|---|---|---|
| mMRC dyspnea grade (0-4) | symptom |
Scoring
0: Breathless with strenuous exercise only. 1: Breathless when hurrying or walking up slight hill. 2: Walks slower than people of same age due to breathlessness. 3: Stops for breath after walking about 100m or after a few minutes on level. 4: Too breathless to leave house or breathless when dressing.
mMRC grade mMRC dyspnea grade (0-4) (0=dyspnea only w/ strenuous exercise; 1=hurrying/slight hill; 2=walks slower than peers; 3=stops after 100m; 4=too breathless to leave house)
Interpretation
mMRC 0-1: Few symptoms (GOLD Group A). mMRC ≥2: Many symptoms (GOLD Group B or E). mMRC ≥2 also triggers pulmonary rehabilitation referral consideration.

How Scribeable handles it in the note
When an encounter activates the Modified Medical Research Council Dyspnea Scale (its highest tier is recommended), Scribeable does not ask you for missing inputs. The score is computed in code from whatever inputs resolve from the transcript, the pasted chart context or charted labs; a computed value is written into the draft only when every required input already resolves.
The score is computed in code from the resolved inputs, not recalled by the language model. If the draft states a Modified Medical Research Council Dyspnea Scale value that disagrees with the computed one, the disagreement is flagged to the second drafting pass, which is instructed to correct the score and its interpretation before you see the note.
Classified as a risk score: activated strictly, by a documented trigger, never by a permissive reading of the encounter.
