7 items, each scored 0-10 on NRS: pain intensity, disability in ADLs, disability in social activities, anxiety, depression, fear-avoidance (work/social), pain locus of control. Total score 0-70.
Interpretation
Score <25: Mild disability — conservative management, activity modification, reassurance. Score 25-50: Moderate disability — structured rehabilitation program, consider psychosocial interventions for fear-avoidance. Score >50: Severe disability — multidisciplinary pain management, psychological intervention essential, address catastrophizing and kinesiophobia. MCID: ~18 points (or 30% change). Captures biopsychosocial model dimensions in a single tool. Available in separate versions for low back pain and neck pain.
