Score <25: Mild disability — conservative management, activity modification, reassurance. Score 25-50: Moderate disability — structured rehabilitation program, consider psychosocial interventions for fear-avoidance. Score >50: Severe disability — multidisciplinary pain management, psychological intervention essential, address catastrophizing and kinesiophobia. MCID: ~18 points (or 30% change).
