When it activates

Scoring

0=No symptoms. 1=No significant disability despite symptoms. 2=Slight disability (own affairs without assistance). 3=Moderate disability (requires some help). 4=Moderately severe (unable to walk/attend to needs unassisted). 5=Severe (bed-bound, incontinent, requires constant care). 6=Dead.

Interpretation

mRS 0-1: Excellent outcome (functionally independent). mRS 2: Good outcome (independent but some limitations). mRS 3: Fair (requires assistance). mRS 4-5: Poor outcome (dependent). mRS 6: Death. Commonly used as primary endpoint in stroke trials (favorable outcome typically mRS 0-2).
