When it activates

Scoring

Category 1: No AMD or normal aging changes — few small drusen (<63 microns). Category 2: Early AMD — multiple small drusen, few intermediate drusen (63-124 microns), or mild RPE abnormalities. Category 3: Intermediate AMD — extensive intermediate drusen, at least one large druse (>=125 microns), or geographic atrophy not involving fovea. Category 4: Advanced AMD — geographic atrophy involving the fovea (dry advanced) or choroidal neovascularization/neovascular AMD (wet advanced).

Interpretation

Category 1: No supplement needed — routine eye exams. Category 2: AREDS2 supplements not clearly beneficial — annual monitoring. Category 3: AREDS2 supplementation recommended (25% risk reduction for progression) — vitamin C 500mg, vitamin E 400IU, lutein 10mg, zeaxanthin 2mg, zinc 80mg, copper 2mg. Follow-up every 6-12 months, Amsler grid home monitoring. Category 4: Urgent anti-VEGF therapy for wet AMD; AREDS2 supplements for fellow eye. 5-year progression risk from Category 3 to 4: ~43% without supplements, ~30% with AREDS2.

How Scribeable handles it in the note

When an encounter activates the AREDS AMD Classification (its highest tier is recommended), Scribeable does not ask you for missing inputs. The score is computed in code from whatever inputs resolve from the transcript, the pasted chart context or charted labs; a computed value is written into the draft only when every required input already resolves.

The score is computed in code from the resolved inputs, not recalled by the language model. A stated AREDS AMD Classification value is compared against the computed one when the draft's structured summary carries it; a disagreement found there is flagged to the second drafting pass, which is instructed to correct the score and its interpretation, unless Scribeable has supplied the computed value to that pass, in which case the value the draft writes is checked against the supplied one and a detected difference is shown beside the note for you to adjudicate rather than silently rewritten.

Classified as a staging or classification system: activated strictly, because a wrong stage on the wrong patient is a documentation hazard.
