Ages & Stages Questionnaire (ASQ-3)
Screens developmental milestones across 5 domains to identify children needing further developmental evaluation
- Highest activation tier
- Recommended
- How it is scored
- Scored in code; a stated value is compared only from the structured summary or a supplied value
- Scoring type
- questionnaire
- Care settings
- Outpatient
When it activates
Scribeable activates a calculator from the encounter. The Ages & Stages Questionnaire (ASQ-3) can activate when the transcript or pasted chart context documents any of these:
- Developmental milestone screening
- Developmental delay concern
- Well-child developmental assessment
- Early intervention referral consideration
- Motor or language delay evaluation
A coded diagnosis can also activate it. ICD-10 prefixes carried by the registry: F80, F82, R62.
Inputs the note needs
2 inputs. Each is resolved from the transcript, the pasted chart context and charted labs; at this calculator’s tier a missing input is never requested, and a missing required input keeps the score out of the draft.
| Input | Type | Unit / options | Guidance |
|---|---|---|---|
| ASQ-3 total score across 5 domains (0-300, each domain 0-60) | symptom | ||
| Whether any individual domain score falls below the age-specific cutoff | history |
Scoring
5 domains with 6 items each (scored 10=yes, 5=sometimes, 0=not yet): Communication, Gross Motor, Fine Motor, Problem Solving, Personal-Social. Domain score range 0-60. Total range 0-300. Age-specific cutoffs for each domain (vary by questionnaire interval, 2-60 months).
Interpretation
Above cutoff (all domains): Development on track — provide age-appropriate activities, rescreen at next well-child visit. Near cutoff (within monitoring zone): Close monitoring — provide domain-specific activities, rescreen in 1-2 months. Below cutoff (any domain): Refer for formal developmental evaluation and early intervention assessment. Early intervention (Part C services) available birth-3 years. IDEA Part B for 3-5 years. Most effective screening done at 9, 18, 24, and 30 months per AAP. Sensitivity ~86%, specificity ~85%.

How Scribeable handles it in the note
When an encounter activates the Ages & Stages Questionnaire (ASQ-3) (its highest tier is recommended), Scribeable does not ask you for missing inputs. The score is computed in code from whatever inputs resolve from the transcript, the pasted chart context or charted labs; a computed value is written into the draft only when every required input already resolves.
The score is computed in code from the resolved inputs, not recalled by the language model. A stated Ages & Stages Questionnaire (ASQ-3) value is compared against the computed one when the draft's structured summary carries it; a disagreement found there is flagged to the second drafting pass, which is instructed to correct the score and its interpretation, unless Scribeable has supplied the computed value to that pass, in which case the value the draft writes is checked against the supplied one and a detected difference is shown beside the note for you to adjudicate rather than silently rewritten.
Classified as a questionnaire: a patient-reported or clinician-rated instrument, where a spare activation carries low harm.
