Scoring

1=Very fit, 2=Well, 3=Managing well, 4=Vulnerable, 5=Mildly frail, 6=Moderately frail, 7=Severely frail, 8=Very severely frail, 9=Terminally ill.

Interpretation

1-3: Fit to managing well. 4: Vulnerable — not dependent but slower, symptoms limit activities. 5: Mildly frail — needs help with IADLs. 6: Moderately frail — needs help with ADLs and IADLs. 7: Severely frail — completely dependent for personal care. 8: Very severely frail — approaching end of life. 9: Terminally ill.

When an encounter activates the Clinical Frailty Scale (Rockwood) (its highest tier is recommended), Scribeable does not ask you for missing inputs. The score is computed in code from whatever inputs resolve from the transcript, the pasted chart context or charted labs; a computed value is written into the draft only when every required input already resolves.

The score is computed in code from the resolved inputs, not recalled by the language model. If the draft states a Clinical Frailty Scale (Rockwood) value that disagrees with the computed one, the disagreement is flagged to the second drafting pass, which is instructed to correct the score and its interpretation before you see the note.
