TIMI Risk Score for STEMI
Predicts 30-day mortality in patients with ST-elevation myocardial infarction
- Highest activation tier
- Recommended
- How it is scored
- Scored in code; a detected disagreeing draft value is flagged for correction
- Care settings
- Emergency department, Inpatient
When it activates
Scribeable activates a calculator from the encounter. The TIMI Risk Score for STEMI can activate when the transcript or pasted chart context documents any of these:
- STEMI presentation
- ST-elevation myocardial infarction
- Acute MI risk stratification
- Post-STEMI prognosis
A coded diagnosis can also activate it. ICD-10 prefixes carried by the registry: I21.
Note-type scope: it activates on notes whose subject resolves to cardiology, emergency medicine, critical care, hospital medicine. A note that resolves to another subject does not carry it even when the clinician's specialty matches; a note with no resolvable subject (a generic consultation) falls back to the specialty match.
Inputs the note needs
8 inputs. Each is resolved from the transcript, the pasted chart context and charted labs; at this calculator’s tier a missing input is never requested, and a missing required input keeps the score out of the draft.
| Input | Type | Unit / options | Guidance |
|---|---|---|---|
| Patient age | age | ||
| Diabetes, hypertension, or angina | history | ||
| Systolic BP <100 mmHg | vital | mmHg | |
| Heart rate >100 bpm | vital | bpm | |
| Killip class II-IV | exam | ||
| Weight <67 kg | vital | ||
| Anterior ST elevation or LBBB | exam | ||
| Time to treatment >4 hours | history |
Scoring
Age ≥75 (+3), Age 65-74 (+2), DM/HTN/angina (+1), SBP <100 (+3), HR >100 (+2), Killip II-IV (+2), Weight <67kg (+1), Anterior ST elevation or LBBB (+1), Time to treatment >4h (+1). Max 14.
Age Patient age (≥75=+3, 65-74=+2) + DM/HTN/angina Diabetes, hypertension, or angina + SBP Systolic BP <100 mmHg (<100=+3) + HR Heart rate >100 bpm (>100=+2) + Killip Killip class II-IV (II-IV=+2) + wt<67kg Weight <67 kg + Anterior ST/LBBB Anterior ST elevation or LBBB + time>4h Time to treatment >4 hours
Interpretation
0-3: Low risk (2-4% 30-day mortality). 4-5: Moderate (8-12%). >5: High (>16% mortality).

How Scribeable handles it in the note
When an encounter activates the TIMI Risk Score for STEMI (its highest tier is recommended), Scribeable does not ask you for missing inputs. The score is computed in code from whatever inputs resolve from the transcript, the pasted chart context or charted labs; a computed value is written into the draft only when every required input already resolves.
The score is computed in code from the resolved inputs, not recalled by the language model. If the draft states a TIMI Risk Score for STEMI value that disagrees with the computed one, the disagreement is flagged to the second drafting pass, which is instructed to correct the score and its interpretation before you see the note.
Classified as a risk score: activated strictly, by a documented trigger, never by a permissive reading of the encounter.
