GOLD COPD Classification

Classifies COPD severity by spirometry and symptom burden using the GOLD 2024 framework for treatment guidance

When it activates

Scribeable activates a calculator from the encounter. The GOLD COPD Classification can activate when the transcript or pasted chart context documents any of these:

  • COPD classification
  • COPD severity staging
  • COPD treatment step-up decision
  • New COPD diagnosis
  • COPD exacerbation risk assessment

A coded diagnosis can also activate it. ICD-10 prefixes carried by the registry: J44.

  • COPD J44.0

Note-type scope: it activates on notes whose subject resolves to pulmonology, hospital medicine. A note that resolves to another subject does not carry it even when the clinician's specialty matches; a note with no resolvable subject (a generic consultation) falls back to the specialty match.

Inputs the note needs

3 inputs. Each is resolved from the transcript, the pasted chart context and charted labs; an input marked "asks you" is requested as a clarification when it is undocumented.

InputTypeUnit / optionsGuidance
FEV1 % predictedlab%
Symptom burden: few or many (mMRC 0-1 or CAT <10 = few)symptom
Exacerbation history: 0-1 moderate or ≥2 moderate/≥1 hospitalizedhistory

Reference for licensed clinicians. Where the registry text carries dose thresholds or conversion factors, they are as published in the cited sources. This page is not a prescribing tool and not individualised medical advice; verify against a current drug reference and the current guideline before acting. Medical disclaimer.

Scoring

Spirometric: GOLD 1 (≥80%), GOLD 2 (50-79%), GOLD 3 (30-49%), GOLD 4 (<30%). ABE Groups: A (few symptoms, low exacerbations), B (many symptoms, low exacerbations), E (high exacerbation risk regardless of symptoms).

Formula as the note renders it

Spirometric grade from FEV1% FEV1 % predicted (≥80 GOLD1, 50-79 GOLD2, 30-49 GOLD3, <30 GOLD4); ABE group from symptoms Symptom burden: few or many (mMRC 0-1 or CAT <10 = few) + exacerbations Exacerbation history: 0-1 moderate or ≥2 moderate/≥1 hospitalized

Interpretation

Group A: Bronchodilator PRN. Group B: LABA or LAMA, or LABA+LAMA if highly symptomatic. Group E: LABA+LAMA (± ICS if eos ≥300). Spirometric grade guides severity.

How Scribeable handles it in the note

When an encounter activates the GOLD COPD Classification at the mandatory tier, Scribeable resolves its inputs from the transcript, the pasted chart context and any charted labs, and asks you for a missing input you can supply before the note is finalised. Bedside-exam and separately ordered study values are not requested one input at a time; a calculator that carries a combined clarification question may still put that single question to you.

The score is computed in code from the resolved inputs, not recalled by the language model. If the draft states a GOLD COPD Classification value that disagrees with the computed one, the disagreement is flagged to the second drafting pass, which is instructed to correct the score and its interpretation before you see the note.

Classified as a staging or classification system: activated strictly, because a wrong stage on the wrong patient is a documentation hazard.
