When it activates

Scoring

Class I: Free of disabling seizures (may have auras). Class II: Rare disabling seizures (almost seizure-free). Class III: Worthwhile improvement (>75% reduction). Class IV: No worthwhile improvement (<75% reduction).

Interpretation

Class I: Best outcome, seizure-free or only auras — considered surgical success. Class II: Rare seizures, generally favorable. Class III: Meaningful improvement but persistent seizures. Class IV: Surgery did not achieve meaningful seizure reduction. Approximately 60-70% of temporal lobe epilepsy surgeries achieve Engel Class I.
