STS Risk Score

When it activates

Scribeable activates a calculator from the encounter. The STS Risk Score can activate when the transcript or pasted chart context documents any of these:

• Pre-cardiac surgery risk assessment
• CABG risk estimation
• Valve surgery risk
• TAVR vs SAVR decision

Note-type scope: it activates on notes whose subject resolves to perioperative, cardiology, hospital medicine. A note that resolves to another subject does not carry it even when the clinician's specialty matches; a note with no resolvable subject (a generic consultation) falls back to the specialty match.

Scoring

Requires 40+ patient variables. Use official STS calculator at riskcalc.sts.org.

Interpretation

Low risk: <4% predicted mortality. Intermediate: 4-8%. High: >8%. Prohibitive: predicted mortality or morbidity exceeds benefit. Used for TAVR vs SAVR decisions (TAVR favored in intermediate+ risk).

Scribeable carries the STS Risk Score in its registry for activation and documentation guidance but does not compute it: the inputs live in the external tool, not in the note.

When an encounter activates it, the value written in the note is whatever you state or the model recalls: Scribeable neither computes it nor compares a stated value against a computed one.

Classified as a risk score: activated strictly, by a documented trigger, never by a permissive reading of the encounter.
