When it activates

Scoring
Cells (in 1mm x 1mm slit-lamp beam): Grade 0=<1, Grade 0.5+=1-5, Grade 1+=6-15, Grade 2+=16-25, Grade 3+=26-50, Grade 4+=>50. Flare: Grade 0=none, Grade 1+=faint, Grade 2+=moderate (iris/lens details clear), Grade 3+=marked (iris/lens details hazy), Grade 4+=intense (fibrin or plastic aqueous).
Interpretation
Cells 0-0.5+: Inactive or trace activity — consider taper if on treatment. Cells 1+: Mild activity — may need treatment initiation or adjustment. Cells 2+: Moderate activity — active treatment indicated. Cells 3-4+: Severe activity — aggressive treatment, evaluate for complications (hypopyon, posterior synechiae). Worsening by 2 grades = clinically significant. Flare may persist after cells resolve (chronic protein leak). Activity patterns: acute (<3mo), chronic (>3mo), recurrent (distinct episodes).
