When it activates

Scoring
Stage I: Initial — CAL 1-2mm, bone loss coronal third (<15%), no tooth loss. Stage II: Moderate — CAL 3-4mm, bone loss coronal third (15-33%), no tooth loss. Stage III: Severe — CAL >=5mm, bone loss extending to mid/apical third, <=4 teeth lost. Stage IV: Advanced — CAL >=5mm, bone loss extending to mid/apical third, >=5 teeth lost, masticatory dysfunction. Grade A: Slow progression (<0.25mm/year bone loss). Grade B: Moderate (0.25-1.0mm/year). Grade C: Rapid (>1.0mm/year or specific risk factors like smoking, diabetes).
Interpretation
Stage I-II: Non-surgical therapy (scaling and root planing) usually sufficient. Stage III: Surgical therapy often needed (flaps, regeneration, implants). Stage IV: Complex rehabilitation — multidisciplinary approach, possible full-mouth reconstruction. Grade A: Stable, routine maintenance sufficient. Grade B: Standard risk, 3-4 month recall. Grade C: High risk — aggressive management, smoking cessation, diabetes control essential, 2-3 month recall. Staging determines complexity; grading determines urgency and prognosis.
