When it activates

Scribeable activates a calculator from the encounter. The Sport Concussion Assessment Tool 5 (SCAT5) can activate when the transcript or pasted chart context documents any of these:

  • Concussion assessment
  • Sport-related head injury
  • Post-concussion evaluation
  • Return to play assessment
  • Traumatic brain injury screening

A coded diagnosis can also activate it. ICD-10 prefixes carried by the registry: S06.0.

  • Concussion S06.0X0A

Inputs the note needs

1 input. Each is resolved from the transcript, the pasted chart context and charted labs; at this calculator’s tier a missing input is never requested, and a missing required input keeps the score out of the draft.

Scoring

22 symptoms each rated 0 (none) to 6 (severe): headache, pressure in head, neck pain, nausea/vomiting, dizziness, blurred vision, balance problems, sensitivity to light, sensitivity to noise, feeling slowed down, feeling in a fog, don't feel right, difficulty concentrating, difficulty remembering, fatigue/low energy, confusion, drowsiness, more emotional, irritability, sadness, nervous/anxious, trouble falling asleep. Total symptom severity 0-132.

Interpretation

Symptom severity <10: Typical normal range (baseline). 10-30: Mild symptom burden. 30-60: Moderate symptom burden. >60: Severe symptom burden. No single cutoff confirms concussion — compare to baseline if available. Serial testing most valuable (worsening = concern, improvement = recovery). Full SCAT5 includes cognitive assessment (SAC), neurological screen, and balance exam. Return-to-sport requires complete symptom resolution at rest and with exertion through graduated protocol.

How Scribeable handles it in the note

When an encounter activates the Sport Concussion Assessment Tool 5 (SCAT5) (its highest tier is recommended), Scribeable does not ask you for missing inputs. The score is computed in code from whatever inputs resolve from the transcript, the pasted chart context or charted labs; a computed value is written into the draft only when every required input already resolves.

The score is computed in code from the resolved inputs, not recalled by the language model. A stated Sport Concussion Assessment Tool 5 (SCAT5) value is compared against the computed one when the draft's structured summary carries it; a disagreement found there is flagged to the second drafting pass, which is instructed to correct the score and its interpretation, unless Scribeable has supplied the computed value to that pass, in which case the value the draft writes is checked against the supplied one and a detected difference is shown beside the note for you to adjudicate rather than silently rewritten.

Classified as a risk score: activated strictly, by a documented trigger, never by a permissive reading of the encounter.
