Clinical Calculators

236 Calculators. Scored in Code.

Clinical scores are computed in code from the documented components and audited automatically, and a detected disagreeing stated score is flagged to the second drafting pass, which is instructed to correct it before you see the note, except CHA2DS2-VASc, whose stated score is never overwritten: its disagreement is surfaced for you to adjudicate instead. No switching to a calculator app.

Scored in Code, Built Into the Note Pipeline

From qSOFA to MELD-Na, clinical calculators are scored in code from the documented components, and a detected disagreeing stated score is flagged to the second drafting pass, which is instructed to correct it before the note is finalized, except CHA2DS2-VASc, whose stated score is surfaced for adjudication rather than corrected.

Pre-Stage-2 Correction

Calculator scores are validated after Stage 1 of note generation. When a stated score in the draft is detected and disagrees with the computed one, a correction is injected into the Stage 2 prompt before the final note is produced. CHA2DS2-VASc is the exception: its computed value only surfaces a disagreement for you to adjudicate, and never overwrites the stated score.

  • Surfaces a detected disagreement before the final note
  • Resolves component values from the Stage 1 draft and its structured summary
  • Injects corrections into the Stage 2 generation prompt
  • Sends the corrected value to the second drafting pass

Post-Stage-2 Audit

After the final note is generated, every calculator score the documented components support is re-validated, except the two that score only on an interactive form and the three the product does not compute. The audit results are stored in Firestore for clinical review and quality tracking.

  • Final validation of the computable calculator scores in the completed note
  • Audit results stored in Firestore (extraData.calculatorValidationsJSON)
  • Discrepancies flagged for clinician review
  • Complete audit trail for compliance and quality assurance

22 Point-Sum Scorers

  • Anion Gap, FIB-4, FENA, Child-Pugh, PREVENT 10-year CVD risk (interactive form)

How It Works

Two-Pass Validation for Clinical Accuracy

Calculator validation happens automatically during note generation — no extra steps.

STEP 01

Detect Scores

AI identifies clinical calculators mentioned in the transcript and extracts component values.

STEP 02

Validate Stage 1

Each score the documented components support is recalculated in code, except the two that score only on an interactive form and the three the product does not compute. A detected disagreement is flagged for correction, except CHA2DS2-VASc, whose stated score is surfaced for adjudication rather than corrected.

STEP 03

Correct Stage 2

Corrections for detected disagreements are injected into the Stage 2 prompt, which is instructed to fix them in the final note.

STEP 04

Audit & Store

Final validation runs on the completed note. Results are stored in Firestore for review.

Clinical Calculator Scoring — 236 Code-Scored Calculators

Scribeable computes 236 clinical calculators in code from the documented components rather than trusting a model-recalled value; two of them (PERC and PREVENT) score only on a deliberate interactive form. Whether a computed value is written into the draft depends on the calculator's activation tier and the questionnaire denylist, and a detected disagreeing stated score is flagged for correction: a stated value is extracted from the note text by a score-extraction pattern for the calculators that carry one, or checked against a supplied computed value in the assessment and plan, and otherwise compared only when the draft's structured summary carries it. CHA2DS2-VASc is the exception: its stated score is never overwritten, and a disagreement is surfaced for the clinician to adjudicate. An audit trail is stored for clinical review.

How Calculator Validation Works

  1. Record and dictate the patient encounter as you normally would
  2. AI detects calculator references and scores them from the clinical data
  3. Pre-Stage-2 correction flags a detected disagreeing stated score before the final note
  4. Post-Stage-2 audit validates the final note and stores results in Firestore
