From qSOFA to MELD-Na, clinical calculators are scored in code from the documented components, and a detected disagreeing stated score is flagged to the second drafting pass, which is instructed to correct it before the note is finalized, except CHA2DS2-VASc, whose stated score is surfaced for adjudication rather than corrected.
Calculator scores are validated after Stage 1 of note generation. When a stated score in the draft is detected and disagrees with the computed one, a correction is injected into the Stage 2 prompt before the final note is produced. CHA2DS2-VASc is the exception: its computed value only surfaces a disagreement for you to adjudicate, and never overwrites the stated score.
After the final note is generated, every calculator score the documented components support is re-validated, except the two that score only on an interactive form and the three the product does not compute. The audit results are stored in Firestore for clinical review and quality tracking.

Scribeable computes 236 clinical calculators in code from the documented components rather than trusting a model-recalled value; two of them (PERC and PREVENT) score only on a deliberate interactive form. Whether a computed value is written into the draft depends on the calculator's activation tier and the questionnaire denylist, and a detected disagreeing stated score is flagged for correction: a stated value is extracted from the note text by a score-extraction pattern for the calculators that carry one, or checked against a supplied computed value in the assessment and plan, and otherwise compared only when the draft's structured summary carries it. CHA2DS2-VASc is the exception: its stated score is never overwritten, and a disagreement is surfaced for the clinician to adjudicate. An audit trail is stored for clinical review.
