Your First Note

A step-by-step walkthrough of creating your first clinical note with Scribeable.

01Starting a Recording

When you're ready to record a patient consultation:

  1. Open Scribeable on your iPhone or Apple Watch
  2. Tap the large orange "Record" button
  3. You'll see a waveform indicating audio is being captured
  4. Place your device where it can hear both you and the patient

02During the Consultation

While recording, you can:

  • Conduct your consultation normally — no special speaking pace or structure required
  • Check the recording timer to see elapsed time
  • Pause and resume if needed
  • Add voice markers by saying "Note:" followed by your comment (e.g., "Note: patient is guarding in the right lower quadrant")

There's no time limit on recordings — record as long as you need.

03Choosing Your Note Type

When you stop the recording, select the note type that matches your encounter:

Progress Note

Daily hospital follow-up or office visit

Admission H&P

New inpatient admission

Clinic Note

Standard outpatient visit

ED Provider Note

Emergency department encounter

Consultation

Specialist referral evaluation

Discharge Summary

Hospital discharge

These are just the most common — Scribeable supports 22+ note types total. See Note Types & Templates for the full list.

04Generating the Note

Once you've selected your note type:

  1. Confirm your specialty is set correctly (this affects formatting and content)
  2. Tap "Generate Note"
  3. Wait 30–60 seconds for processing

Processing time depends on recording length. A 15-minute consultation typically processes in under a minute. You'll get a push notification when your note is ready.

05Reviewing Your Note

Once generated, review your note carefully:

  • Check that all key information was captured
  • Verify medication names and dosages
  • Review the Assessment & Plan section
  • Check the AI-suggested ICD-10 codes and billing recommendations
  • Make any necessary edits by tapping on a section

You can also switch to a different note type after generation — your content is preserved, only the structure changes.

Always review AI-generated notes before finalizing. You are responsible for the accuracy of your clinical documentation.

06Exporting to Your EHR

Once you're satisfied with the note:

  • Browser extension: Get a 6-digit code and use the Chrome extension to insert directly into Epic, Cerner, athenahealth, and 12+ other EHRs
  • Copy to clipboard: One tap copies the full note for pasting anywhere
  • PDF export: Generate a formatted PDF for printing or sharing
