The after-visit summary comes from the note you already finished

The visit isn't over when the note is signed. There's the paper the patient takes home, the letter the specialist needs before they'll book the appointment, the note for the employer, and once in a while the disability form that takes longer than the visit did. I used to do all of that by retyping the same facts from the note into four different shapes, and every retype was a chance to get a dose or a date wrong in a document the patient was going to read at their kitchen table.

Why the documents have to come from the note
